Perlman è noto soprattutto per aver recitato nei film del regista Guillermo del Toro, con ruoli in: Hellboy (2004), Hellboy II: The Golden Army (2008), Cronos (1993), Blade II (2002) e Pacific Rim (2013).
Perlman è anche conosciuto per aver interpretato Clay Morrow nella serie televisiva Sons of Anarchy (2008-2013) e Vincent nella serie televisiva Beauty and the Beast (1987–1990), per la quale ha vinto un Golden Globe.
